Informed consent and release of liability 

Parents/Carers and children attending classes (referred to below as participants) and  future classes of The story Frog agree to: 

 

  1. Participants agree to abide by the rules and regulations of the class and the use of equipment, facilities and services. 

  2. Participation in activities by children and adults involving equipment such as scissors, other objects or equipment that may cause harm is understood to be undertaken under the supervision of the parent or carer also participating. 

  3. Participation in all activities shall be undertaken at the participants own risk and the teacher shall not be liable to the participant for any claims of injury, damage or causes of action to person, property arising out of or connected to participation in the classes. 

  4. Participants agree that s/he is emotionally and physically able to use the equipment, facilities and to take part in activities offered. 

  5. Participants understand and acknowledge that their child’s safety is their responsibility. 

  6. Participants will inform the class leader of any allergies or medical conditions that their child has and although we always, where possible label activities which contain allergens we cannot guarantee this and therefore require participants to make sure that each activity is suitable for their child should they have a specific allergy or medical condition. 

COVID 19 Guidance: 

The attendee understands their responsibilities when attending The Story Frog classes to help prevent the spread of Coronavirus infection by:

●      Ensuring they use hand sanitiser on entry to the sessions

●      Ensuring they use hand sanitiser between activities where necessary and if their child or themselves touch their face

●      Minimising the number of adults who attends sessions with a child (maximum of 1)

●      Securing their place in this bubble group by booking their space on an ongoing basis

 

Avoid spreading the virus by ensuring: Anyone who has coronavirus (COVID-19) symptoms, such as a raised temperature, a new continuous cough, or a loss or change to their sense of smell or taste, should not attend an out-of-school setting and should follow the COVID-19: guidance for households with possible coronavirus (COVID-19) infection.
